This version adds the \areadoconhecimento macro, in order to include its content in the monography cover. This is a new requirement from the 2018 edition of the university formatting guidelines. The macros \avaliadori, \avaliadorii, \avaliadorext were also added, so that the arguments of the \imprimirfolhadeaprovacao macro were simplified. A starred version of the \incluirimagem macro was added, so that no caption is added. The layout of the cover, title page and approval page were also changed. Aditionally, the \icomma package was added, and the page is formatted with the \raggedbottom command, instead of \flushbottom.
